1. Student Information:
   • Include your name, student ID, programme (MPhil/PhD), and contact details.
2. Supervisor Information:
   • Clearly state the name, designation, and contact details of your Supervisor.
   • Acknowledge your Supervisor as the Chair of the Advisory Committee.
3. Advisory Committee Members:
   • List the names and designations of other members proposed for the Advisory Committee.
   • Collaborate with your Supervisor to finalise the committee members.
4. Committee Roles:
